ENROLLMENT INCLUDES:

1. Unlimited extended office visits (At least 30 minutes EVERY visit).

2. Unlimited phone, text, e-mail, and videochat visits when clinically appropriate.

3. One comprehensive physical per year to review your overall health and discuss ways to improve and maintain your health over time.

4. One Well Woman Exam per year (when applicable) including comprehensive women’s health review, breast exam, pre-pregnancy counseling or counseling on birth control options, and pap smear collection at appropriate intervals.*

5. Basic office procedures including skin tag removal, freeze treatment for warts and pre-cancerous skin lesions, diabetic nail trimming, removal of moles, excision of simple skin cysts, incision and drainage of simple skin abscesses.*

6. After hours face-to-face visits in the evenings and on weekends are also available for urgent issues for established patients only. After hours phone/text/email visits for urgent concerns are included with membership.

7. Contraception services (birth control), weight loss services, assistance with smoking cessation and many other services are included as well.

8. Access to our negotiated discount rates for labs and imaging at local labs and imaging centers.

9. Access to wholesale pricing on medications provided directly through our office. We do NOT stock or dispense controlled medications at all, and do not routinely prescribe or refill controlled substance medications for long term use. This includes medications for anxiety (like xanax and ativan), ADD/ADHD (like vyvanse, concerta, focalin, daytrana or adderall), narcotic pain meds (like percocet, vicodin, hydrocodone, etc) and insomnia (like ambien, lunesta, restoril, etc).

*Lab fees for blood testing, imaging/x-rays, pap smear and skin biopsy tissue analysis (when needed) will be charged by the lab or imaging facility to your insurance. If you do not have insurance or choose not to use insurance to cover these, I have negotiated discount rates which are plainly listed upfront. Procedures that require additional supplies, medications or equipment may also be a small extra fee which would be our wholesale cost for these supplies, medications or equipment plus a small percentage to cover bottles, labels, and supplies.

NOTE: We will turn off our cell phones during protected family times- 1 hour each evening for family dinner time, school programs and recitals, church, meditation time, etc; and will return any urgent calls or texts as soon as possible. We will also close the office completely for two separate weeks each year to take a break, spend time with family, unplug, relax and replenish. These weeks will be announced in advance, and we will do our best to try to coordinate care from another physician in or near the sproutMD office during our absence, but this may not always be possible. We may also rarely be out of the immediate area on weekends and unavailable for in-person visits at those times but will still be accessible by phone/text/e-mail and videochat. You will need to use your best judgement regarding how to handle any urgent or emergency concerns should we not be available. We are striving to begin caring for ourselves better so we can better care for all those who need us, for many years into the future!

Texting and email are protected on our end, but are considered “non-secure” methods of communication. If you wish to be able to communicate this way, you will need to authorize this during your enrollment process. Please be cautious about any photos, personal information, or private concerns that you may share via text or email as they may show up on your cell screen when we reply.

MEMBERSHIP FEES:

ENROLLMENT FEE (one time fee at enrollment) is equivalent to one month membership by age category. After you register as a new patient, your membership will then be individually confirmed, and you will be notified by e-mail. Your enrollment fee and first month membership will be billed at the time of enrollment.  If you choose to enroll mid-month, a prorated fee will be added to your billing to cover the remainder of that month until the next regular billing cycle on the first of the next month.

  • age 18 and under: $40 per month
  • age 19-45:  $60 per month
  • age 46-64: $70 per month
  • age 65 and up: $80 per month

Billing processes on the first of each month for that month’s services and will auto-renew each month until you cancel. There are also options to pay 3, 6, or 12 months at a time. All memberships automatically renew each cycle unless you cancel in advance.

We use a secure EMR (electronic medical record) which is called AtlasMD. You can find out more about this EMR built specifically for DPC practices at AtlasMD EMR info. Our billing processes through the EMR as well. Billing may show up on your bank statement or credit card as ATLASMD- WICHITA, KANSAS, or MEDICAL SERVICES- KANSAS, or STRIPE – KANSAS. If you are unsure of a charge, please contact us. If charges are disputed, billing fees are added by the billing system and will be charged back to the payment account.

There is a minimum 3 month membership for children age 18 and under, and a minimum of one full month membership for adults (a partial/prorated month does not qualify).

There is no fee for termination or cancellation. If you feel that the DPC model or sproutMD are not right for you, cancel at any time without penalties. Any remaining full months you paid in advance will be refunded in full.
